The name that designates a company within the American Modern group. |

Number assigned to a particular contract by American Modern. |

Current condition of coverage - shows whether or not coverage is in force. |
Entered Date |
Date the current information was entered. |

Identification number of the user who entered the current information. |
Policyholder's Name |
First and last name of the policyholder. |
Address |
The address of the insured property. |
Mailing Address |
The complete house, street, apartment/unit, or Post Office Box number which is used by American Modern to mail all communication to the insured. |
Home Phone |
Phone number, including area code, of the insured's home. |
Work Phone |
Phone number for insured at work. |
Date of Birth |
Date of birth of the policyholder. |
Social Security Number |
The Social Security Number of the insured. |
Original Effective Date |
Date the policy was first place with American Modern. |

Amount Due Now |
Total pending policy premium currently due, plus service and other charges; sum of all pending charges: (Payment Amount + Service Charge + Other Charges). |
Service Charge

Amount that is added to the amount due when a payment plan is used. This is the amount that reimburses the company for the expense incurred in preparing and mailing multiple bills and posting multiple payments. |
Other Charges |
Other charges, such as NSF and Reinstatement/Lapse Charges. |
Total Outstanding |
Total outstanding premium balance due on policy, plus any outstanding service and other charges: New Premium Balance + (Sum of all Outstanding Service Charges + Other Charges). Same as Remaining Balance on the billing notice. |
Unpaid Balance |
Amount of the current term premium that has not been paid. |
Total Premium Received |
Amount of the current term premium that has been received and posted to this policy. |
Reason for Bill |
The activity that caused a bill to generate. |
Earn Premium Date |
The date to which premium dollars that have been paid are earned. For example, a policy that is effective on January 1 with a premium of $500 of which $250 has been paid has an earned premium date of July 1. |
Last Payment |
Payment amount posted by American Modern. |
Date Posted |
Date the payment is entered into the American Modern system. |
Concurrent Billing |
Information on the most recent billing activity. |
Next Bill Date |
This date is calculated based on the billing cycle and the effective date of the policy term. |
Next Bill Amount |
The dollar amount that will appear on the next bill. |
Billing Plan |
The Billing Plan for the policy. |
Bill To |
The category of party to whom Bills will be sent (e.g., Agent). |
Payment Plan
|
A schedule, selected by the insured, within which bills are sent to the insured and payments are due to American Modern. |
Billing Status |
Brief information regarding recent billing activity. |
Pending Payment |
Indicates if a payment has been made but not yet applied to the policy. For a value of Yes, click the link to view the payment details on the Billing History page. |

The date that the policy coverage becomes effective. |
Expiration Date |
The date that a policy term ends. |
Written Premium |
The sum of all premiums from the policy and endorsements written during a specific time period. |
Full-Term Premium |
The total premium charge for all coverages on a policy during an entire policy term. |
Agent
|
An insurance company representative licensed by the state who solicits, negotiates or effects contracts of insurance, and provides service to the policyholder for the insurer. If the mouse pointer is held over the agency number, the agency name is displayed. |
Agent Type -
|
Indicates the agent's channel of distribution. Some examples are L=Lender, D=Dealer, G=General Agent, I=Independent Agent. |
Subproducer
|
An agent who has a contractual relationship with another, primary agent or managing general agent to place risks with a carrier through that primary agent. |
Number of Liens |
The number of lienholders that have an interest on the policy. |
Total Claims |
The number of claims that have been reported on the policy. |
Number of Open Claims |
The number of claims that are currently open on the policy. |
