Client Information

Adding/updating clients (Contract Holders, Sub-Producing Organizations, Licensed Producers or Processors), requires a modernLINK Profile Administrator security profile.
Any text entered during a setup that is printed on a Dec page is printed as-is; be very careful with spelling and use of upper and lower case.

The Client Information page provides a place for you to enter information that identifies the entity you are setting up or updating. Some fields are pre-filled with information initially entered on the setup page, or, if a matching entity was found, that was selected from the Possible Match list.

To enter Client Information:

  1. Complete the Client Information forms for a business or individual. Click the to add additional address, phone, or e-mail entries. To update information, click Edit on the page bottom, then click Modify to select a section for update. In modify mode, click Add to List to add another entry.

Business Client

Client Information Business fields:

Business Name

The name of the business.

 

DBA

The "doing business as" name as indicated on the license of a Contract Holder or Sub-Producing Organization.

 

Tax ID#

The business's federal tax identification number.

Address

Type

Defaults to Office for a business type contract holder. Select Mail Address only if mail is not sent to the  office address.

 

Mailable?

Indicates if the Postal Service has deemed the address as mailable or not.

 

Attention / Care of

Indicates special handling of mail.

 

Adress1

Street address for the selected address.

 

Address2

Suite number or additional address info for the selected address.

 

Zip Code

Zip code for the selected address.

 

Country

Country for the selected address, U.S.A. or Canada.

Phone(s)

Type

Defaults to Office Phone for a business type contract holder or SPO. Other options include Home Phone, Mobile Phone and Office Fax.

 

Area Code

Area code for the selected phone.

 

Phone

Number for the selected phone.

 

Ext.

Extension for the selected phone.

 

Phone # listed?

Indicates if phone number for the selected phone is listed or not, for privacy purposes.

e-Mail Address

Type

Defaults to Office for a business type contract holder or SPO. Home is the other option.

General e-mail address of the business. For a Contract Holder, the office e-mail address receives electronic notification of appointments.

Commission Payments (EFT)

Bank Name

Name of the Contract Holder's bank for deposit of commission checks.

 

Account Number

Account number at Contract Holder's bank for deposit of commission checks.

 

ABA Number  (Routing Number)

ABA/Routing Number of the Contract Holder's bank for deposit of commission checks.

Premium Payments (EFT)

Bank Name

Name of the business's bank from which American Modern sweeps premium payments.

 

Account Number

Account number at business's bank from which American Modern sweeps premium payments.

ABA/Routing Number of the business's bank from which American Modern sweeps premium payments.

Bank account type, savings or checking.

  1. Click Save to Continue, Cancel to abort the setup/change.

If Save is clicked, the setup menu opens showing the Client Information section is complete. For a change, the information is updated and saved. A change to Client Information does not have to be submitted. For some changes, such as changing the business name of a Contract Holder, American Modern Licensing & Contracts is notified to finalize the change.
