Insurance Score Request

The Insurance Score Request page provides a place for you to enter information to be used to order the insured's credit-based insurance score that is used as part of the underwriting evaluation or rating.

To enter Insurance Score information:

  1. Complete the Insurance Score Request form.

Ensure that the insurance score request disclaimer is read to the applicant.

Select check box to indicate that you have read the disclaimer to the applicant to approve ordering of the Insurance Score Report.

First Name

The applicant's first name, carried over from Basic Information.

Middle Initial

The applicant's middle initial, carried over from Basic Information.

Last Name

The applicant's last name, carried over from Basic Information.

Social Security Number

The applicant's social security number.

Date of Birth

The applicant's date of birth.

Is mailing address same as location address?

If Yes, mailing fields are autofilled.

Mailing Address

The applicant's mailing street address.

Mailing City

The applicant's mailing address city.

Mailing State

The applicant's mailing address state.

Mailing Zip Code

The applicant's mailing ZIP code.

  1. Click Next.

The result is one of two possibilities:

A) If an insurance score is pulled and results in the applicant not getting the best possible rate, you are notified and advised to print the Fair Credit Reporting Act (FCRA) letter. This letter is a legally-required notice to the client that they did not receive the best possible rate.

B) If the FCRA letter is not required, the Coverages page opens.

  1. If the applicant's insurance score prevents the best possible rate, print the FCRA letter and give or mail it to the applicant. Select the Print page menu command and then click FCRA Letter. Note that the letter is attached to the quote sheet and the application and prints when these two items are printed. The quote can be saved for future reference if required.
